A. "It claims 0.3% is a universal physical law." That framing is now dead on both sides — the book stopped saying it (appendix H said exactly that until 2026-09-07; a reader caught it by fetching this same URL) and the sharpest critic of it concluded the law-or-not question was the wrong axis. The claim is that kE is a UNIT: a crossing is what 0.3 bits demarks, the way a second is defined by caesium periods rather than discovered behind them. So ask the better question instead: when the text cites a 0.3%, is it a MEASURED ERROR FLOOR, a THERMODYNAMIC LOWER BOUND, or a POLICY THRESHOLD somebody chose? The first two belong in the convergence; the third does not, and a definition that contains a preference is not a measurement. Find me any place the book still lets the third sit with the first two.
B. "It claims error correction does not exist." Does it? Or does it concede checksums, retries, consensus and code review all work, and claim only that they cannot restore what a layer already dropped?
C. "It claims only hardware can audit anything." Does it? Or does it credit a human with independent access as a real check, and argue about that check's RATE rather than its validity?
D. THE OPEN ONE, and I want your honest read rather than a defence. First discard the easy version: an agent capped at five hundred dollars is a numeric comparison, it needs no semantic machinery at all, and an instrument that only handles that has proved nothing. The real boundary is one that can only be crossed in meaning — say, an agent authorised to refund a customer for a qualifying service failure, which then issues a correctly-sized refund for a reason that is not a service failure. The amount is fine. The justification drifted. Can the instrument take a boundary of THAT kind, stated in my words, and place a crossing of it? Or must a human first compile my policy into its lattice, which is where the hard judgement would be hiding? If the second, it is a classifier with a philosophy attached. Say which the text actually supports, and say if the text does not settle it.

Four days now. He read it and decided not to answer, or he read it and forgot, or it went to promotions, or it never left my outbox at all — and I am going to spend Thursday deciding which, on no evidence, in my head.Nobody answered and nothing looked are two different facts about the world, and every instrument you own reports them to you with the same blank screen. That blank is the cheapest-looking line item you carry and one of the most expensive: the reply you stopped chasing, the feed that died in August, the agent whose morning report came back clean because the check never ran at all. A silence is a finding — somebody had the message and chose. A blind spot is the absence of a finding, and it is not a weaker version of the first thing, it is a different thing entirely.
Your inbox, your dashboard, your test suite and your agent's own summary render both of them as nothing, and not one of them is lying to you. They were built with one exit code where two were always needed, and the second one costs four lines.

The repair is not a better search. It is a second exit code.
exit 1 means "I looked, and there was nothing there."
exit 2 means "I could not look."
So the first question is not whether they answered. It is whether anything looked, and the honest follow-up is one almost nobody in any building can answer on the spot. When did the thing you are staring at last successfully reach the source it claims to be reading — not the last time you opened it, the last time it looked?
Most of the time, nothing looked.
This is measured rather than imagined, and the number is ours before it is anybody else's. On 2026-08-30 this repository audited its own message store and found the iMessage feed had failed 111 consecutive scheduled ticks since 23 August, and the inbound-email feed 206 — while every query anyone typed against that store came back instantly, confidently, and wrong by omission. Nobody was told, because nothing was built to tell them. Two channels dark and one healthy is strictly worse than three channels dark: the live feed makes the whole record feel current, which is the same reason a partially working smoke detector is more dangerous than none. What shipped was not a better sync. It was a door that exits non-zero while any channel is blind, so an absence can never again be handed over as a silence.
The claim at full strength, with the falsifier attached, because it is cheap to hand over. No account of a process, produced by that process, can contain what the process never observed — which is why "the report came back clean" is a sentence about the report and never about the world. A log is written from inside the room it describes; a receipt is a recomputation over a record the actor did not author, which is why the check must read the immutable commit and not the working tree the actor still holds a pen over. The falsifier is one sentence: produce an account, authored by an agent, that distinguishes "I found nothing" from "I never looked" using only what that agent wrote down. Run npx thetacog-mcp attest-demo on your own machine — a one-minute local run that reads nothing of yours and sends nothing anywhere — and the thing worth watching is not the verdict. It is that the run refuses to answer at all when it could not read what it needed. That refusal is the entire product, and it is four lines of code that nobody writes, because a function that returns nothing found passes every test you would think to write for it and ships feeling finished.
Here is the concession before the argument, stated harder than a hostile reader would state it. We shipped this exact bug, more than once, in the code that exists to catch it. A receipt writer here reported success to every caller for twenty-one hours against a ledger that never moved, because a shell builtin swallowed the write; the silence was read as a dead loop and the loop was fine. A guard written to catch a defect was run against a scratch tree with all eleven of its assertions green and never touched the defective path at all. A false green is indistinguishable from a real one from the outside, which is the whole difficulty and the reason this is a physics claim rather than a discipline problem.
A working smoke detector and a detector with the battery out give you the same information on a quiet night, and the quiet night is 364 days out of 365. You do not find out which one you own on the quiet nights. You find out on the one night the answer matters, which is the one night it is too late to check.

Two results close two different doors, and a critic has to break both. Rice proved in 1953 that no machine decides a non-trivial property of what another program computes, which closes the door marked was it good — permanently, for every reviewer model anybody will ever ship, because a larger one is a better storyteller inside the same failure domain. The second door is quieter and it is the one this post is about: an account of a process, produced by that process, cannot contain what the process displaced or never observed. Model capability is the wrong dial for both. One door is shut by decidability and the other by information, and the two results have nothing to do with each other, which is exactly why the pair holds.
The consequence people find surprising is that tamper-resistance is not the test. A perfectly honest, cryptographically signed, immutable log written by the actor is still an emission from inside the boundary — it contains what the actor noticed, and its silences are the actor's silences. A receipt is a recomputation over a record the actor did not author. That is a question about which side of the boundary the artifact was produced on, and it has a clean answer for every artifact you own, which is more than can be said for most audit questions.
None of this is new and that is the point worth carrying. Luca Pacioli published double-entry bookkeeping in 1494 and it still runs the accounting profession, not because the arithmetic is clever but because the counter-entry is written by somebody the bookkeeper does not employ. Double-entry is adversarial, not arithmetical. The date on the milk is useful precisely because the milk did not print it.

You have done this recently and you were not careless when you did it. The dashboard that went flat and read as a quiet week, when the exporter had stopped scraping. The test that reported green because it was skipped, and the skip was printed in grey. The candidate who ghosted, whose reply sat in a spam folder nobody has a habit of opening. The alert that never fired, from a rule whose count(*) = 0 is produced by an index that is still rebuilding rather than by a quiet system. The direct message the app swore it had sent, in a client that has been failing its background refresh since 23 August.
In every one of those the instrument handed you a single word — nothing — and you did the only thing available to a person holding one word. You inferred intent, because intent is the interesting explanation and because nothing in the interface offered you the boring one. That inference is not a character flaw and treating it as one is the reason staleness alerting keeps getting proposed and keeps not getting built: it gets framed as vigilance, and vigilance is a thing you run out of by Thursday.
The starter died on Tuesday. Every loaf since has looked exactly like dough, and every one of them was a report from inside a bowl that had stopped fermenting. The reason you cannot catch this by being careful is that there is nothing to catch — no error, no red, no exception, no line in a log. There is only a blank, and blanks are what a careful person is worst at, because a careful person reads a blank as information.

Here is the whole of it, and it is yours to use with no attribution and no software. Before we argue about why nobody replied — when did this thing last successfully look? In a terminal that is usually one command: node scripts/chat/messages-db.mjs --cutoffs is the shape of it here, and every stack has an equivalent that nobody has ever typed.
Watch what that question does to a room, because the effect is reliable enough to plan around. It is not an accusation, so nobody defends. It is answerable in principle, so it does not read as rhetoric. And it is almost never answerable in practice, which means the room discovers together that it has been reasoning about intent from an instrument that was never asked whether it could see. The person who asks it is not the sceptic in the meeting; they are the one who found the cheapest bug in the building. A stale feed costs one query to detect and can burn a quarter before anybody notices, which is the highest ratio of any bug class we have measured here.
It also survives being asked about your own work, which is the test of whether a question is a gift or a weapon. When did our own check last successfully look? That is the question that produced the 111 and the 206, and the reason both numbers are printed in this post rather than fixed quietly. A silent hole with a measured size is a receipt. A fixed bug is a shrug.

The repair is small enough to be embarrassing. A checker returns exit 0 when it looked and everything was fine, exit 1 when it looked and found something, and exit 2 when it could not look at all. Three states, one extra branch, and the class of failure this post is about becomes structurally unrepresentable rather than merely rare. That is the whole engineering content, and its cost is measured in lines rather than in architecture.
It is running here in three places and each one earns its keep differently. The page tracer exits 2 when nothing is listening on the host, because I did not look and I looked and saw nothing are different claims about a page. The message door exits non-zero while any channel's cutoff is older than the question being asked, and it withholds the verdict rather than serving a confident partial answer. And the boundary probe refuses its own reading when the control condition falls outside a plus-or-minus 15 percent band, printing NOT ADMISSIBLE instead of a number, because a measurement taken on a machine that was too busy is not a weaker measurement, it is not one.
The part that took longest to learn is that a count beats a boolean. A feed that is one tick behind and a feed that has been dead for a month both report stale, and the words are identical while the situations are not. So the door prints a consecutive-failure streak, and 111 is a sentence that a single word could never have said. The first number tells you something is wrong. The second tells you how long you have been wrong, and that is the one that changes what anybody does about it.

The strongest objection is not that this is wrong. It is that it is small: you have dressed an operations-hygiene problem in epistemology, every mature team already alerts on feed staleness, and a heartbeat check is a solved 2011 problem you have written a thousand words about. That is a good objection, it is roughly half right, and it should be said out loud in any room where this is being sold.
Here is the half that survives. Staleness alerting covers the feeds somebody thought to instrument, and the feeds nobody instrumented are not a random sample — they are precisely the ones that have never reported a problem, because a component that has never reported a problem is the last one anybody adds a heartbeat to. The selection is the mechanism: the quantity is unmeasured exactly where it has always been silent. And the newer surface has no 2011 practice behind it at all, because an agent's own summary of its own work is a feed nobody thinks of as a feed. Nobody puts a heartbeat on a paragraph of prose.
And here is the ugly half, which belongs in the same course rather than in a footnote. We have an obvious motive for wanting this to be a large problem, and you should price that in. We sell an instrument whose entire function is making this class countable, so read the 111 and the 206 as what they are — a measurement we took on ourselves, on a specific date, which you can go and reproduce on your own machine and which would have been much more comfortable to leave unpublished.

The sufficiency contract is printed on every verdict this system produces, and it is short enough to quote in full. WHERE a piece of work landed is provable and re-runnable — WHETHER it is bug-free is undecidable, by Rice, and we do not claim it. That fence is not modesty and it is not a disclaimer bolted on by a lawyer. It is the reason the first half is worth anything: a system that also claimed the second half would be claiming something a 1953 theorem forbids, and one reader noticing that ends the credibility of everything beside it.
So the scale is turned toward you. The look either happened or it did not, and that is a fact about a machine at a timestamp rather than a judgement about anybody's work. Whether the reply you eventually got was worth having is not something any instrument decides, ours included. Whether your agent's output was correct is not decidable and nobody selling you a reviewer model has escaped that. What is decidable is whether the check ran, when it last saw the source, and where the result was placed against a lane that was declared before the run started.
We never guarantee behaviour. Insurance never prevented a single fire; it made fire a number, and the number is what let anyone build in wood. The four verbs are that the deviation is detected, placed, priced and dispatched — every one of them checkable, none of them a promise about conduct. And the count is deliberately cause-blind: a hidden trigger, a prompt injection, a stale feed and a plain mistake all produce the same crossing and the same tick, so nobody has to win the attribution argument before the event is countable.

In January the exclusions arrived on paper. CG 40 47, CG 40 48 and CG 35 08, ISO and Verisk, effective 2026-01-01, landing on general liability rather than on cyber. We cannot tell you what any individual underwriter will decide and we will not pretend to — what we can tell you is what was filed, and a filed form number is not something a conversation talks its way past. You can deploy. What you cannot do is deploy covered, and the difference between those two sentences is the whole of what changed this year.
Which means the desk is not the one people aim at. Your CISO cannot sign for this and should not be asked to — they are measured on incidents prevented, and a deployment that turns out to be uninsurable is not an incident, it is a renewal. It lands on counsel, on the risk committee, on whoever answers the board when the board asks whether the AI work can be covered. That is also where the independence requirement already lives: Sarbanes-Oxley and the EU AI Act converge, from completely different directions, on the same sentence — the insured cannot certify its own innocence — which is the boundary argument from course B wearing a compliance suit.
The redirect costs one sentence and it is worth writing down. This is not a security ask and your CISO is the wrong signature — it belongs with whoever owns the exposure the exclusion actually lands on. Say that, and the security team becomes the person who forwards it rather than the person defending a budget they were never going to win.

What is on the record, handed over as ingredients rather than as a verdict. Rice's theorem, 1953, for the undecidability half. Landauer, 1961, for the cost of erasing a record, confirmed experimentally in Nature 483:187 in 2012. Cover and Thomas, Theorem 2.8.1 — the data processing inequality — for the part that says no reconstruction procedure recovers what a channel discarded, which is the formal shape of a report cannot contain what nobody observed.
