Whenever you perform causal reasoning, you encounter a fundamental challenge: every "why" can be met with another "why." Why did the project fail? Because of a missed deadline. Why was the deadline missed? Because a key dependency was late. Why was it late? This branching chain of questions creates a combinatorial explosion of possible causes.
