fix(bookclub): sink runs must NOT write the live idempotency ledger

ROOT CAUSE of the 2026-07-14 skip incident: the individual-send loop appended to
data/bookclub/sends/<day>.progress.ndjson on ANY r.ok — including SINK (offline-proof)
runs, which 'succeed' per recipient without sending a real email. The immortal-hold
guard test's stale-clear case ran an (unthrottled) sink send that wrote ~1500 fake
'done' rows; the real live send then SKIPPED 1067 recipients who never got the email.
Fix: gate the progress append on LIVE — the ledger records REAL delivery only. Also
hardened the guard test to snapshot+restore the receipt AND progress ledger (a sink
receipt could otherwise un-complete a real day and re-trigger a send). Guards:
tests/ops/bookclub-hold.test.mjs (LIVE-gated progress · non-destructive test).
NOTE: recovery re-send to the ~1507 wrongly-skipped recipients is in flight as this lands.

fix(bookclub): sink runs must NOT write the live idempotency ledger

ROOT CAUSE of the 2026-07-14 skip incident: the individual-send loop appended to
data/bookclub/sends/<day>.progress.ndjson on ANY r.ok — including SINK (offline-proof)
runs, which 'succeed' per recipient without sending a real email. The immortal-hold
guard test's stale-clear case ran an (unthrottled) sink send that wrote ~1500 fake
'done' rows; the real live send then SKIPPED 1067 recipients who never got the email.
Fix: gate the progress append on LIVE — the ledger records REAL delivery only. Also
hardened the guard test to snapshot+restore the receipt AND progress ledger (a sink
receipt could otherwise un-complete a real day and re-trigger a send). Guards:
tests/ops/bookclub-hold.test.mjs (LIVE-gated progress · non-destructive test).
NOTE: recovery re-send to the ~1507 wrongly-skipped recipients is in flight as this lands.
